Essential Job Functions:
 
 1. Work independently in the field, and as a member of a team to ensure the safety and well-being of all residents receiving services
 
  • Regularly assess, correct and report room and personal living space inspections conditions as assigned
  • Attend case conference meetings for residents as assigned
  • Participate in meetings as assigned including supervisory and service planning meetings
  • Facilitate resident empowerment committees/activities
  • Assist residents with the safe handling of food and preparation of meals as needed
  • Assist residents in the organization and maintenance of their apartments and personal living space.
  • Assist in the organization and maintenance of common areas where indicated
  • Participate in a 24/7 crisis response rotation, as indicated
  • Adhere diligently to all life-safety policies and procedures
 2. Perform all duties in a manner that is appreciative of residents’ strengths and supportive of their continued recovery.
 
  • Be informed of resident’s backgrounds, histories and assessments from information contained in available records
  • Encourage residents to function independently through prompts and role modeling
  • Assist residents in identifying and creating as many natural supports as possible
  • Acknowledge success and engage residents in collaborative problem solving
 3. Provide Case Management for a specified group of residents:
 
  • Engage them in setting rehabilitation and housing support goals, and collaborate with them in developing a Housing Support Plan where indicated
  • At the time of admission of a new resident, complete an admission note
  • Facilitate and coordinate their access to services, resources (e.g. educational, vocational, spiritual, social) and recovery-based supports both within, and external to, SUS
  • Develop a service plan for all new residents within 28 days of admission, and     annually thereafter
  • Teach them skills that will permit them maximal independence in maintaining their residence, their income and their employment , if applicable
  • Complete service plan reviews on a quarterly basis for all residents, with the      initial review occurring three months from the date of admission.
  • As needed, assist them in the management and maintenance of personal belongings and a safe living space
  • As needed, serve as an advocate for residents in obtaining benefits, managing and coordinating their medical and psychiatric treatment and rehabilitation, and overcoming stigma and other barriers to community integration
  • Maintain up to date, accurate, complete and timely documentation (handwritten and electronic word processing) of resident progress in individual client records.
  • Support, guide and counsel residents towards self-management and self advocacy in all areas of functioning
  • Complete the Utilization Review form for all clients based on a monthly schedule.
  • Adjust resident’s goals, objectives, methods, and target dates as necessary
  • Assist residents in maintaining independence in the self administration of medication
  • Provide crisis intervention and management as needed
  • Adhere diligently to all service delivery policies and procedures
  • Ensure the uniformity of all case records
4Any other duties as assigned by the Program Director, designee or as required.
 

The successful candidate must have a 4-year degree in Mental Health (MH) field from an accredited institution and experience in MH in a group home setting.
