Among your responsibilities, s/he will:
 
·         Work closely with Director and Assistant director in identifying medical and psychiatric needs of 276 consumers in scattered site housing.
·         Work closely with Case Managers/ Service Coordinators in meeting the varied needs of consumers
·         Attend protocol meetings and assist in developing plans of action for consumers in “crisis’
·         Visit consumers in their homes to assist with medical concerns.
·         Assist consumers in understanding medical conditions and provide support and teaching when appropriate.
·         Advocate for consumers with medical/psychiatric providers.
·         Attend case conferences in hospitals to assist with safe discharge plans.
·         Perform Nursing assessments for new admissions and, when able, on other consumers.
·         Develop and provide in-service education for team members (i.e. infection control, diabetes, hypertension etc.)
·         Develop relationships with local clinics and hospitals
·         Participate in Evidence Based Practice Trainings and implementation.
 
               
Congregate Sites
·         Work closely with Directors and Assistant directors of each residence in identifying medical or psychiatric needs of consumers.
·         Assist consumers in understanding medical conditions and provide support and teaching when needed.
·         Work closely with Medication coordinators at each residence
·         Assist all staff with medical/ psychiatric advocacy
·         Review MAR sheets and provide assistance in working with consumers to achieve medication adherence.
·         Attend Case conferences to assure safe discharge planning
·         Prepare and provide in-services for staff
·         Prepare and provide in-services for consumers
·         Perform nursing assessments
·         Work towards providing Metabolic Monitoring for all consumers
·         Recommend medical intervention when needed and assist with implementation
·         Advocate with medical/ psychiatric providers
·         Work with VNS Congregate Care Nurses in each residence
·         Assist staff with referrals to VNS, Clinics etc
·         Review and evaluate medical information and assist consumers in understanding results of tests, new medications, medical or psychiatric recommendations etc.
·         Provide 24 hour phone access for questions/ advice
·         Supervise part-time nurses when applicable
