Registered Nurse (Aids Services)

 

 

 

Reporting to the Project Director, the successful candidate will work with small caseloads within the framework of a team to provide basic nursing services as well as health education and treatment adherence counseling to a small caseload of targeted individuals diagnosed with HIV/AIDS who are also diagnosed with a mental illness and/or substance abuse disorder.
  

This position focuses primarily on the provision of Health and Psycho-Educational services, as well as Group and Individual Counseling.

Among your responsibilities, you will:
·     Train project staff, residential staff, and program participants on health issues including general health, substance abuse, mental health, HIV/AIDS, diabetes, Hepatitis, cancers, STDs, weight management, nutrition, and other diseases which may affect particular participants etc. in both individual and group settings
·     Stay current on health, mental health, and substance abuse issues including new medications and other treatments
·     Conduct Wellness Self Management groups
·     Conduct Base line Screening and Assessments to determine the participants health status, risk factors and lifestyle issues
·     Meet regularly with each participant to assess health status
·     Develop relationships with local mental health, health and substance abuse providers to facilitate referral, treatment retention and care coordination
·     Make the appropriate referrals to community based medical care
·     Advocate on behalf of participants regarding medical, mental health, substance abuse treatment as needed
·     Provide health management education
·     Review treatment and medication history
·     Provide medication education and provide reminders to participants who have difficulty with medication treatment adherence
·     Provide a range of treatment, rehabilitation, and support services to the participants
·     Responsible for documentation of services provided particularly as related to the Individual Wellness Plan
 
 
The successful candidate must have:
·     A valid license to practice Nursing in New York State
·     Atleast 3 years of professional clinical nursing experience including at least 1 year of experience in providing direct services to individuals diagnosed with mental illness and/or substance abuse disorders
·     Excellent communication skills and strong computer literacy
·     Commitment to working flexibly to meet client's needs
·     The ability to work effectively in a multi disciplinary team setting and demonstrate a commitment to further development with the support of the team
 
 
Preferred Credentials and Skills
.   A Bachelor Degree or a Masters Degree in Social Work or Counseling
·     Supervisory experience and experience with people with HIV/AIDS and community Nursing experience
·     Bilingual in English and Spanish
·     A solid understanding of recovery-oriented service delivery
